In order to protect you and others we are asking about symptoms and exposure
to COVID-19. Your health is our priority please answer these questions
so that we can direct you to the care that you need as quickly as possible.

- Fever >100
- Cough
- Shortness of Breath
- Sore Throat
- Chills
- Muscle Aches/Pain
- New Loss of taste or smell
- Have you been around anyone that has tested positive for COVID-19 in the
last 14 days?
- Have you tested positive for COVID-19 in the last 14 days?
For a pre-op appointment or well child checks for patients under 3 years
of age please call the office to schedule.

APPOINTMENT CONFIRMATION
Your safety is our number one concern, so please read the following information
about our safety measures and our request of you.
- Please wear a mask to your appointment. If you do not have one, we will
gladly provide one.
- On the day of your appointment, if you have a temperature of 100 degrees
or higher, a cough, difficulty breathing, sore throat, chills, muscle
aches or pain, diarrhea or vomiting, loss of taste or smell, or have been
around anyone that has tested positive for COVID19 or you yourself has
tested positive in the last 14 days, please contact our office for further
screening prior to coming for your appointment.
- You will be screened upon arrival to the office with temperature check,
symptom check and to ensure you have brought a mask.
- You may also be contacted closer to your appointment if your reason for
visit is more suitable for a telehealth visit.
Adult Patients: If you are scheduled for an in-person visit, please note
our new policies to keep you and others safe:
- To best protect our patients and employees, anyone age 2 and above is required
to bring their own mask or cloth face covering to wear over their nose
& mouth during their visit.
- We are NOT allowing any visitors at this time. If you need assistance,
one person may accompany you and will need to follow the same guidelines.
Pediatric Patients: If your child is scheduled for an in-person visit,
please note our new policies to keep you and others safe.
- To best protect our patients and visitors, anyone age 2 and above is required
to bring their own mask or cloth face covering to wear over their nose
& mouth during their visit.
- ONLY ONE parent or caregiver should accompany the patient.

A caregiver from your doctor’s office will call you
15-20 mins before your appointment to provide a
Zoom meeting ID that will be required to join the meeting.
- If you do not receive any call before your appointment time, please call
your doctor’s office.
- If you’ve provided a cell phone number, you will be sent instructions
on how to connect to Zoom for the telehealth visit.

If you are a new patient, please complete the new patient paperwork prior
to your first visit. Choose your medical group:
