Accepted Health Plans
Let's make the process of finding health care coverage less complicated.
Before scheduling your appointment, we
strongly recommend you contact your insurance company to verify that the medical group location or provider you plan to visit
is included in your network. Your insurance company will also be able
to inform you of any copayments, co–insurance, or deductibles that
will be your responsibility. If you proceed in scheduling an appointment
and your health insurance benefits do not participate with St. Joseph
Heritage Healthare, your out of pocket liability may be higher.
Orange County and High Desert
Medical groups include Mission, St. Joseph and St. Jude Heritage Medical
Groups, and Mission, St. Joseph and St. Jude Affiliated Physicians.
Northern California
Medical groups include St. Mary High Desert Medical Group and Premier Healthcare.

participation is subject to change at the sole discretion of the health
plan. Please contact the health plan directly to confirm the status of
your medical group current participation in a particular provider network
and/or product which may or may not be listed above.
