Jaclyn Staubach is a Licensed Clinical Social Worker who provides collaborative care services as part of the Behavioral Health & Primary Care Integration program for Providence St Jude Heritage Medical Group. Jaclyn completed her undergraduate degree at California State University, Fullerton and went on to obtain her Master of Social Work degree at Azusa Pacific University. Prior to her current role, Jaclyn worked in different settings within the Providence health system family. She worked at St Jude Medical Center as a palliative care social worker supporting patients and their families as they navigated serious and life-limiting illnesses, as well as at St Jude Neighborhood Health Centers as a mental health therapist.
Her approach to treatment integrates therapeutic modalities such as Cognitive Behavioral Therapy and Solution-Focused Therapy. Many of her interventions focus on helping patients increase self-awareness, as well as introducing skills and tools to help manage their mental health symptoms. Jaclyn strives to motivate and empower patients to take an active role in their therapy to help them reach their therapeutic goals.
