    Love this care provider and just learned she is leaving at the end of this month.. :-(
    My provider is knowledgable and listens with great care. She encourages my participation in the care plan.
    Very sad that she is being transferred to another medical office. I will miss her. She has been one of the best providers that I have had at providence.
    I've always had good experience with Tracey Bloom
    I'm the caregiver for the patient seen by Tracy Bloom. My mother has always has excellent care from Tracy and the staff with this practice. We are sad that Tracy is leaving the group but we have chosen to stay with this medical group because of the wonderful care they have all provided. We are taking Tracy's recommendation of another primary care within the same family practice group. We are very hopeful and confident in this decision.

    Biography
    Tracy Bloom is a board certified Nurse Practitioner in Acute Care and Family Medicine. She completed her Bachelors of Science in Nursing at D’Youville College in Buffalo, New York in 1998. She graduated from Wayne State University in 2000 with a Master’s of Science in Nursing with the certification of Acute Care Nurse Practitioner. In 2015 she completed a Post-Master’s certification as a Family Nurse Practitioner at Sonoma State University. Tracy has held other Nursing positions during her 30 year career including serving in the United States Air Force from 1985 to 1995 as a flight nurse and delivering compassionate care to patients at the end of their lives through her work with Napa Valley Hospice. For the last 16 years, Tracy has practiced at Queen of the Valley Medical Center as an Acute Care Nurse Practitioner specializing in the field of Cardiology and managing hospitalized patients with significant cardiac disorders including arrhythmias, heart failure, and myocardial infarctions. In November of 2019, she made the transition to the outpatient medical practice to focus on disease prevention and looks forward to serving the families in Napa with all their healthcare needs.
